If you are paying by Direct Debit complete Direct Debit Instruction, please note we can not accept weekly payments by Direct Debit

If you are paying through your payroll complete Employee Payroll Deduction Mandate

If you are unable to print and scan the form please email us at apps@tfs.uk.com providing your full name and we will send you a copy in the post for you to complete. Please note this will delay the processing of the application.
